Planned Extended Absence Form

All students planning to be away for more than 3 consecutive schools days due to vacation or other personal reasons not relating to illness or bereavement, must have this form filled out. 

Please have this signed by all of the student's teachers, the student, a parent/guardian and brought back to the Main Office for Vice Principal Approval.
